    Systems and methods for improved radiation treatment planning can employ hierarchical constrained optimization with relaxed constraints also referred to herein after as expedited hierarchical constrained optimization (ECHO). The systems and methods described herein employ a combination of constrained optimization and a correction loop involving unconstrained optimization to enhance the speed and efficiency of the radiation treatment planning process and improve the accuracy and quality of resulting radiation treatment plans. The use of a hierarchical constrained optimization approach leads to less complex and faster to solve optimization problems. The correction loop allows for compensating for optimization error associated with the sequence of constrained optimizations by incorporating such error in an unconstrained optimization. Hard and soft dose volume constraints can be efficiently incorporated.

    In order to estimate a radiation load quantity for an imaging system having a radiation source, a base model is obtained for determining the radiation load quantity depending on a set of input variables, which include at least one radiation-source operating parameter. Measurement data measured for the imaging system is obtained that includes the radiation load quantity for a multiplicity of different values of the at least one radiation-source operating parameter. A refined model is generated by adapting the base model depending on the measurement data. Actual values of the set of input variables are defined for the imaging system. The refined model is used to determine, depending on the actual values of the set of input variables, an estimated value of the radiation load quantity.

The system generally includes an ultrasound device placed within the esophagus to image and map cardiac structures in real-time. The system may also control the ventilation of the patient to optimize ultrasound monitoring and radiation delivery. The device in the esophagus is designed to position and monitor the esophagus and/or nearby structures to optimize radiation delivery to targets while minimizing radiation to other key structures. In addition, different ablation technologies may be delivered from within the esophagus to ablate certain tissues that cannot be safely ablated with radiation therapy. Finally, the esophageal device may have electrical or magnetic properties that can be used to guide the radiation therapy.
